COMMUNITY NURSE / CARE COORDINATOR

JOB TITLE: Community Staff Nurse / Care Coordinator
TYPE OF CONTRACT: Part Time ? Permanent.
NUMBER OF HOURS: **Apply on the website** hours a Week With some On
Call Responsibility.
LOCATION: Based in Cork City with some travel to services in the
county.

THE ROLE

WHAT WE DO?
A Care Coordinator role is to support staff and management team, to
include an element of training and supervision. This position involves
leading a team who support individuals in all aspects of their lives,
supporting the Service Manager /CNM1 to effectively manage services
delivered to individuals in Cheshire Ireland Services.

HOW WE WORK.
The Cheshire Community Living Service is based on clear principles of
supporting people ?one person at a time?. As a Care Coordinator
you will have responsibility for the assessment, planning, delivery
and evaluation of care required to meet the physical, psychological
and social needs of persons with physical and neurological
disabilities.

WHAT YOU WILL DO.
The successful candidate will have the ability to coordinate and
liaise with the multi-disciplinary team in response to identified
needs of individuals receiving a Cheshire Ireland service. Actively
contribute to improve the quality of care through implementation of
Best Possible Health (BPH) and HIQA standards. Act as a clinical
support resource and carry out clinical interventions where required.

SKILLS AND EXPERIENCE

* In possession of a nursing qualification and registered with An
Bord Altranais/NMBI
* A minimum if **Apply on the website**-months post qualification
experience.
* Will hold a full clean driving licence and be willing to use their
vehicle for work.
* Experience of working in a care delivery capacity, preferably
within a disability service or community setting.
* Evidence based decision making skills.
* Mentoring and line management skills.
